CalVax Grant Fact Sheet July 22, 2021 California Department of Public Health, CalVaxGrant, COVID-19, medical practice, myCAvax, physician, Physicians for a Healthy California 2832 OVERVIEW Funded by California Department of Public Health (CDPH) and administered by Physicians for a Healthy California (PHC), the CalVaxGrant practice support grants will help physician medical organizations (with 200 or less physicians) administer COVID-19 vaccines in their community, as well as reimburse for related expenses incurred or to be incurred between November 1, 2020 and November 1, 2021. Allowable expenses include staffing, training, technology, infrastructure, supplies/equipment and administrative overhead. Organizations can apply to receive $10,000 for up to five vaccination sites for a total of $50,000. Additionally, applications can receive an additional $1,000 per site (up to five sites for an additional $5,000) if their vaccination sites match high priority areas as noted by the CDPH. ELIGIBILITY + Have a physician medical practice with no more than 200 physicians + Have an approved Section A and B of myCAvax o Need help with the myCAvax application? Primary care practices can email practice and contact information to vaccinenetwork@cmadocs.org for one-on-one support + Be an eligible myCAvax provider type: Health center (community non-Federally Qualified Health Center/non-Rural Health Clinic, migrant or refugee, STD/HIV clinic, student), Indian Health Service, Medical practice (family medicine, internal medicine, OB/GYN, other specialty, pediatrics), Public health provider (FQHC) or Tribal health + Expend or plan to expend award on allowable expenses + Have not already received pandemic-related grant funding GRANT CYCLE CalVaxGrant has approximately $40 million to award 3,000-4,000 applicants on a first come, first served basis!
